 One at a Time Sample Starter Kit:
Please call 800-252-5954 or send an e-mail to henry@quietheartmusic.com to receive a complete Sample Starter Kit. This will include 6 CDs along with 6 matching cards for only $30 plus shipping. (+ PA sales tax, if applicable.) There is no additional “set-up” fee. All CD covers & matching cards will be personalized according to your expressed preferences and will serve as your reference for possible future orders. These personalized imprints also serve as a “draft”, so changes can be made if you so desire. Your personalized card & cover templates will be kept on file indefinitely. Requested changes can be made at any time at no cost. Ordering the Sample Starter Kit does not obligate you to participate in this program. (Limit: 1 Sample Starter Kit per Organization available at this introductory price; sorry, only professional requests can be honored). An invoice will be included with the Sample Starter Kit.
How To Order “One-at-a-Time”:
Click on Order Forms to place your order. Just print & fax the appropriate forms. Alternately you can e-mail the equivalent information to henry@quietheartmusic.com or place your order by calling 1-800-252-5954. You can also request the optional Sample Starter Kit, preferably by phone. To order the Sample Starter Kit, you will be asked to provide the following information:
- Your name, job description & work phone#
- Your Facility or Organization's name and address
- The Designated Giver's Name; i.e. The Staff & Residents / of Lake View Home.

Regardless of how you choose to order, please always include the following 3 items:
- Name of Deceased Resident(s)
- CD title(s)
- Name & Address of Family Recipient(s)
(Name of Address of recipient is optional since you may choose to have the gift CD sent directly to you)
You may choose to order individually as deaths occur, or in groups—monthly, quarterly, or for a annual service of remembrance. The choice is yours--continue your participation only as long as you are 100% satisfied!
Most orders are shipped the same or next day!
How to Pay:
Invoices are generally faxed or e-mailed when the balance exceeds $100. For example, 6 individual orders = $110.70 with standard bubble mailers or $119.70 with deluxe matching mailers. Any additional "copies" of "standard" gifts ordered cost only $10 each; additional copies of "deluxe" gifts cost $11.50 each. You can either bill to a credit card on file or pay by check.
